    - Departmental Enquiry
   - Information from Doctor/Hospital    - Death Certificate
   - Original Policy Bond    - Application for transfer of code to nominee
   - Copy of Newspaper having description of accident

    - It is mandatory for the client to be admitted to a government hospital or registered nursing home for at
     least 24 hours in the case of mediclaim. 
    - Your insurance will be effective after 15 days of date of joining as the member.
  
INSURANCE OF 1 LACK
Affect your income anyway: only the insurance policy will lapse automatically. As soon as your commission
exceeds Rs. 3000 the sum of Rs. 300 for renewed will automatically be deducted from you commission.
